The Role The Senior Service Coordinator provides leadership and operational oversight in the coordination of home care and community-based services to support older people to live safely and independently in their homes. This role is responsible for ensuring service delivery is timely, responsive, and aligned with individual client needs, while meeting all regulatory, quality, and organisational standards.Working closely with Care Managers, service providers, and clients, the Senior Service Coordinator facilitates effective communication across all stakeholders, oversees workflow and service requests, and ensures accurate and timely documentation in client systems. The role is also accountable for incident and risk reporting through platforms such as ionMy, monitoring compliance with Aged Care Quality Standards (ACQSC), and supporting a culture of continuous improvement and client satisfaction.This role provides guidance to service coordination staff, contributes to performance reporting, and supports the operational readiness of the service.
Key result areas: • Coordinating services to clients in their home. • Respond to service requests within 24 hours • Manage and facilitate the transfer of information across all stakeholders efficiently. • Reporting in ionMy (risk management system) and investigations of incidents, hazards, near misses and complaints within 24 hours • Maintain data timely in the Client Management System including attached documents, assessments, notes or external reports • Weekly and monthly reports • Client satisfaction and team player across Home Care • ACQSC compliance What We’re Looking For Qualifications Highly Desirable: - Ability to speak multiple languages
Desirable: - Certificate IV or above in Aged Care, Community Services, Business Administration
or Leadership
Experience - Previous experience in rostering/scheduling within aged care, community care or similar environment
- Strong leadership skills with experience mentoring a small team
- Excellent communication and interpersonal skills
- High level of attention to detail and ability to manage competing priorities
- Sound knowledge of relevant aged care legislation, standards, and workforce awards
- Proficiency in client and rostering systems (e.g., AlayaCare,)
- Ability to remain calm and solutions-focused under pressure
- Ability to accept direction and provide timely and constructive reporting
- Ability to work with minimal supervision
