The Role The Community Nurse is responsible for delivering high-quality clinical care and comprehensive assessments to older clients living at home under the Support at Home program. The role focuses on promoting independence, wellbeing, and safe ageing in place through evidence-based nursing practice, care planning, and collaboration with care partners and multidisciplinary teams. Ensuring Care is delivered in accordance with established clinical guidelines, organisational policies, and the Aged Care Quality Standards and legislative framework, ensuring safety, dignity, and optimal wellbeing for every client. Responsibilities Clinical Assessment & Care Planning - Conduct comprehensive clinical assessments (initial, routine, and review) in clients’ homes
- Identify health needs, risks, and goals in collaboration with clients and/or their representatives
- Develop, implement, and evaluate person-centred care plans
- Undertake clinical risk assessments (e.g. falls risk, skin integrity, nutrition, cognition)
- Monitor changes in client health status and adjust care plans accordingly
Clinical Care Delivery - Provide direct nursing care including (but not limited to):
- Wound care and management
- Medication management and administration
- Chronic disease management (e.g. diabetes, cardiovascular conditions)
- Continence assessments and support
- Palliative and end-of-life care (where required)
- Ensure all care delivered aligns with best practice and clinical guidelines
Care Coordination & Collaboration - Work collaboratively with Care Partners, allied health professionals, GPs, and external providers
- Support referrals to appropriate services based on clinical needs
- Participate in case conferences and multidisciplinary reviews
- Provide clinical guidance and support to care staff
Documentation & Compliance - Maintain accurate, timely, and compliant clinical documentation in client management systems (e.g. AlayaCare)
- Ensure all care aligns with aged care standards, organisational policies, and regulatory requirements
- Contribute to audits, incident reporting, and continuous improvement activities
Client & Family Education - Educate clients and families on health conditions, treatments, and self-management strategies
- Promote health literacy and preventative care
- Support clients to make informed decisions about their care
Quality & Safety - Identify and report clinical risks, incidents, and concerns
- Participate in quality improvement initiatives
- Uphold infection prevention and control standards
- Ensure safe medication practices
What We’re Looking For Qualifications - Bachelor of Nursing, Current APRA Registration
- Current First Aid and CPR certification.
- Valid driver’s licence and access to a reliable vehicle.
Experience Broad clinical background as a Registered Nurse in a Community of Residential Aged Care Setting with 2 plus years’ experience Experience working with persons living with dementia, pain management and palliative care highly regarded
Essential skills - Exceptional verbal communication, negotiation, and writing skills
- Excellent customer service, organisational and planning skills
- The ability to prioritise during times of competing demands.
- Eye for detail and very high standards of accuracy
- Ability to work with others and be a participative and supportive team member
- Ability to accept direction and provide timely and constructive reporting
- Competency in Microsoft Programs including word, excel and outlook
- Knowledge and experience with managing data in Client Management Systems
- The ability to work in a culturally sensitive manner within an ethno-specific environment.
- A preparedness to work to the 10 principles of the Eden Alternative philosophy.
- Self-motivated with a flexible, caring and patient attitude.
Desirable - Experience working within Support at Home or CSHP program
- Knowledge of client management systems such as Alaya Care
- Post Graduate qualification in gerontology, community health or chronic disease management
- Ability to speak a second language other than English
- Current First Aid and CPR certificate
- Roadworthy and comprehensively insured vehicle that can be used for work purposes
- Travel across MiCare locations, interstate and overnight stays may be required at short notice
