The Role The Care partner understands activities that contribute to the overall safety, wellbeing, health and quality of life of an older person. Care management ensures that care reflects individual preferences and culturally appropriate practices, while assisting clients to understand and access the level of support they require. The role will support all clients re receiving services under all Support at Home classifications. This includes self-managed clients who require a base level of care management in order for the provider to meet their obligations. Care management involves initial and ongoing care planning, service planning and coordination, development, review and evaluation of a client’s care plan and quarterly budget, monitoring of services provided, provision of information and education about the client’s care and services, support to connect to health and other services when needed, support to undergo a Support Plan Review, if needed. What We’re Looking For Key Result Areas - Improved aged care outcomes - provision of high-quality, person-centred care to all clients
- Enhanced continuity of care – ensure communication, collaboration and smooth transitions between providers and care settings
- Increased client empowerment and satisfaction - engage client’s and their representatives in decisions regarding their care in a timely and supportive manner
- Optimised use of resources – Ensure consolidated service planning and information sharing
- Business development – Encouraging the use of MiCare services with existing and new clients where this makes the most sense for their care
Qualifications - Tertiary, ideally Bachelor level, qualification in Social Work, Allied Health, or Nursing
- Current APHRA registration would be highly regarded
Experience - Demonstrated case management experience within the sector, preferably working with complex family and social issues.
- Thorough understanding of, and empathy for, the aging process, aged care systems
and opportunities.
